Release and Waiver of Liability

I hereby attest that I am fully cognizant of the risks involved in participating in the MOST VALUABLE KIDS (the “Foundation”) Activity offered at the MVK COMBINE EVENT (the “Activity”), including but not limited to risks brought about by physical exertion, and agree that my child(ren) and I will abide by and adhere to the published safety rules of the Foundation and those of the BULLIS SCHOOL as applicable.  I further attest that neither my child(ren) nor I suffer from any heart, lung, or other medical condition or disease that might in any way hinder or prevent me from fully participating in the Activity.

In consideration of my child(ren)’s and/or my participation in the Activity, I hereby release, discharge and hold harmless the Foundation and its respective officers, directors, employees, members, subsidiaries, agents, successors, and assigns, including, but not limited to, any vendor or third party working in conjunction with the Foundation to put on the Activity, from any and all liability that may arise, directly or indirectly, now or in the future, by reason of any injury, damage, loss, or expense incurred in connection with my participation in the Activity, including that caused solely or in part by the fault (including but not limited to negligence, gross negligence, and/or recklessness) of the above-named parties. This Release and Waiver of Liability shall be binding on my heirs, executors, administrators, successors, and assigns, and all minors under the age of eighteen (18) participating in the Activity named below. I agree that if any portion of this Release and Waiver of Liability is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

I HAVE READ AND UNDERSTOOD THIS RELEASE AND ENTER INTO IT IN CONSIDERATION OF THE OPPORTUNITY FOR MY CHILD(REN) AND/OR ME TO PARTICIPATE IN THE MVK COMBINE EVENT ACTIVITY.
